Job summary
The
post holder will be the Consultant Pharmacist for Gastroenterology at The Mid
Yorkshire Teaching NHS Trust (MYT) and will work with other local healthcare
economy organisations to influence the strategic management of patients with gastrointestinal
conditions across the Wakefield and North Kirklees community.
The
post holder will communicate in appropriate ways, highly complex information
about medicines used for the treatment of gastrointestinal conditions to
consultants and other clinicians, general practitioners, pharmacists, nurses,
patients and carers in order to achieve the optimum use of this group of
medicines.
The job plan for this post holder will encompass approximately 4 sessions of expert clinical practice, 2 sessions of professional leadership and consultancy, and the remaining 2 sessions shared across research, teaching, and audit.

Main duties of the job
Support the Consultant Gastroenterologists and specialist nurses working in Gastroenterology in developing a Trust-wide approach to medicines optimisation for patients with gastrointestinal conditions.
Be responsible with the Clinical Lead for Gastroenterology for managing and implementing Trust strategy for improving medicines prescribing for gastrointestinal conditions throughout MYT.
Enhance links and collaboration between Pharmacy and the Department of Gastroenterology within the Trust.
Contribute to the wider healthcare community's management of medicines used for gastrointestinal conditions and influence Pharmacy practice.
Develop a Trust strategy for education and training in relation to medicines prescribing, design curriculum, plan and deliver specialist training on medicines optimisation to a wide range of healthcare providers.
Provide expert advice at highly specialist level on pharmaceutical matters relating to medicines used for gastrointestinal conditions and support the development of guidelines in the use of these medicines.
Lead, plan and undertake clinical audit and research in relation to improving medicines prescribing.
Provide an outpatient specific Consultant-level service
Provide leadership on medicines optimisation for patients with gastrointestinal conditions, at a regional and national level for primary and secondary care.
Provide education and training on gastrointestinal medicines to trust, local higher educational institutes and beyond.

compliance with medicines legislation and avoid errors and side effects.
To be an active and
accessible member of the MDT, participate in joint rounds with the
Gastroenterology team, and provide highly specialist advice to senior
clinicians on drug treatments for individual patients, applying critical
analysis skills, professional knowledge and judgement where information is
lacking or opinion is divided to influence medical consultants on the
management of pharmaceutically complex cases.
Be able to conduct an open discussion when advice is challenged.

As an Independent
Prescriber, provide dose adjustment, advice and prescriptions for medicines for
gastrointestinal conditions and liaise with MDT colleagues where their
expertise or support is required to achieve the best therapy for patients.
Maintain appropriate databases, including recording relevant biochemistry
results in patients notes. Recording action plans, and adjusting doses to
account for renal or liver dysfunction.
Maintaining
appropriate records for independent Prescribing. Taking full responsibility for the
prescribing (writing and signing) of prescriptions as necessary.
