Clinical Pharmacist plus EDC

broaden your experience of hospital Pharmacy, whilst contributing towards the
work of a busy department, then this is the job for you. We are pleased
to offer an opportunity for an enthusiastic clinical pharmacist to join the
pharmacy team at Mid Yorkshire Hospitals NHS Trust.
You will be part of our progressive pharmacy team and
contribute to the implementation of the Medicines Optimisation and Pharmacy
Services strategy, striving for excellence to provide the highest standards of
care for our patients. The successful applicant will join our pharmacy team working within Care of the Elderly.
In addition to excellent communication and clinical skills, you will have previous experience of working within a clinical environment. You
would have completed, or be near to completion, of a post-graduate diploma in clinical pharmacy in order to support
your work as a specialist clinical pharmacist.
You will contribute to the weekend, late night and
emergency duty rotas.
You will also be encouraged to undertake the Non-Medical
Prescribing course if you have not already done so.

Main duties of the job
To deliver a
high quality specialist clinical pharmacy service on a rotational basis.
To work with the Advanced Clinical Pharmacist(s) in the specific clinical
specialty area, and support them in service development and audit.
To undertake
routine clinical pharmacy duties in designated areas of work, both clinical and
operational and ensure compliance with medicines legislation.
To support pharmacy
service areas in the department if required.
This may include dispensaries, technical services and medicines
information.
Departmental
duties will require the individual where appropriate to take a clinical
supervisory role.
To undertake
the professional duties of a pharmacist within the Trust's service.
To ensure,
under the guidance of the relevant pharmacy manager, appropriate supervision of
pharmacy technicians, pharmacy support workers and trainee pharmacists and to
provide mentoring of junior pharmacists in order to maintain strict compliance
with checking and working procedures.

The post holder is accountable for his/her own
professional actions. Guidance of senior colleagues will be available for
reference if required but the post holder will have independent responsibility
for all duties carried out on a day to day basis.
Provide pharmaceutical care to patients at ward
level in both the acute hospital and in organisations with which the Trust has
a Service Level Agreement such as Intermediate Care and Community Services.
This role involves the development and maintenance of care plans for patients
on the unit/directorate, including:
- taking a
medication history and completing medicines reconciliation,
- assessing patients
own medicines,
- reviewing
medication regimens to ensure safe effective and cost-efficient use,
- appropriate
monitoring of treatment given during the hospital stay,
- developing
plans for care and education of the patient about treatment changes and the
best way in which they can manage their own medicines, where appropriate
through self administration schemes,
- facilitating
the discharge planning process and promote seamless care across the
primary/secondary care interface,
- facilitating
implementation of local and national guidelines for medicines use,
- liaising
with staff over issues of storage and stock control and monitor and review
stock holding, and
- reporting
medicines related incidents and complaints with a view to resolving problems
and minimising risk.
Provide evidence based pharmaceutical advice to
the medical and nursing staff. This may be active - where pharmaceutical
problems identified by the post holder require resolution, or passive - where
doctors or nurses seek advice from the pharmacist.
Carry out duties in the dispensary to help to
ensure that medicines are provided in a timely fashion for in-patients and
out-patients. These duties will include:
