Job summary

The specialist palliative care team have an exciting opportunity to recruit a passionate individual who has experience of working with adult patients with a life limiting illness.

The role is varied and includes supporting outpatient clinics at the Rosewood Centre- the main clinics are oncology and palliative care clinics. Patients who attend the oncology clinics are curative / non-curative.

The role will also include working within the wider specialist palliative care team.

In order to be considered for this post, you must hold the relevant UK NMC registration or be in a position to successfully apply for this at the point of your application. Job responsibilities

Act as a role model within Specialist Palliative Care, both within the Trust and externally.

Facilitate care by providing clinical assessment. Determine priorities of care and advise on basic symptom control within your scope of practice, referring to the CNS for more complex care, emotional, psychological, spiritual and social care.

Contribute towards a multi-professional approach in the management of patients

Work collaboratively with primary care colleagues through the implementation of and attendance at Gold Standards Framework meetings, other associated forums .and shared governance initiatives.

Work in partnership with other health care and Specialist Palliative Care providers such as care homes and hospices, referring appropriately and facilitating a seamless service for patients.

Establish and maintain effective communication with patients and carers/relatives.

Demonstrate a basic knowledge in relation to pattern of disease, markers of disease progression and understand the range of treatment available.

Demonstrate knowledge and awareness of drugs used for the control of symptoms in palliative care.

Develop own clinical knowledge and participate in the development of others' clinical knowledge.

Undertake clinical supervision of nursing colleagues on an individual or group basis as appropriate to banding.

Provide appropriate support and advice to bereaved carers known to the Specialist Palliative Care service. Refer on to local bereavement services as appropriate.

Act as a core team member of the Specialist Palliative Care MDT.

Recognise ethical and legal issues, which have implications for nursing practice and take appropriate actions.

Promote the use of nationally recognised end of life care tools to improve the quality of care.

Continually monitor standards of care and participate in the improvement of care through benchmarking, clinical audit and research.

Evaluate and disseminate research findings and facilitate evidence-based practice
