Job summary
The Integrated Transfer of Care Hub (ITOC) have an exciting
new role, we are looking to recruit a Discharge Support Officer to work
alongside the Multidisciplinary Team. You will have delegated responsibility
for facilitating the delivery of safe, effective, and timely discharge/transfer
of patients from Mid Yorkshire Teaching Hospitals into the community or other hospital/community
facilities. These maybe both simple and
complex.
The post holder will
be able to make judgement involving facts or situation which may require
analysis e.g. alter programmes of activities or making decisions related to
specific pathway of care, whilst acting as a central point of contact for
discharge planning.
We are looking for an energetic individual who has a sound
background of the health and social care environment to coordinate discharges
and work within the multi-disciplinary team.
This is a seven-day service that provides single point of
contact across all wards and hospital sites within Mid Yorkshire Teaching Hospitals
Trust for staff, patients, families, carers and associated people and
co-ordinate / contribute to the safe and timely hospital discharge in partnership
with other multi-disciplinary colleagues.
Please note that this role is not eligible for
sponsorship under the Skilled Worker route.

Main duties of the job

To line manage a small team of Administrative Staff with in ITOC.
To work with the discharge sisters and discharge
coordinator to promote the correct implementation of discharge processes
throughout Mid Yorkshire hospitals to ensure safe and timely discharge and
smooth transfer of patients.
Actively participate in discharge planning and
feel confident in ensuring that discharge activity is occurring.
Provide concise and detailed information regarding
patients discharge plans.
Work closely with all internal and external
partners and agencies to ensure a seamless service, including working to reduce
delayed discharges.
To promote best practice in achieving high
quality discharge planning and support in reducing No Reason to Reside numbers.
To use tact and diplomacy where there is dispute
over complex issues regarding leaving hospital and there may be disputes
regarding courses of action to be taken.
To liaise with relevant staff and agencies on a
daily basis to resolve daily operational issues and ensure discharges are
managed effectively.
Acts as an advocate for patients and their
families/carers recognising the boundaries of their clinical knowledge;
liaising and referring on to other services / agencies as required.
To understand and apply the skills and knowledge
around complex needs and discharge planning.
