patient safety. The role will support the Quality, Risk & Governance Lead
Midwife and the governance team along with the senior leadership team to manage
and coordinate all aspects of clinical risk and governance. The post holder
will support and work with the team to develop, implement, monitor, review and
embed strategies and frameworks to underpin the principles of clinical
governance in Midwifery Services; this will include effectively managing
clinical risks. The post holder, with support and oversight from the senior
leadership team will work with the MDT and lead on the enaction of Saving
Babies Lives workstreams and compliance with Maternity Incentive Scheme, ensuring
the timely completion of safety actions and collation of supportive evidence.

Main duties of the job
The post holder will
support the management of incidents, serious incidents, and formal internal
investigations via PSIRF frameworks, promote good practice in risk management
and provide advice on the protection of patients, staff, visitors, and the Trusts
assets. A key role is to ensure that the organisation learns lessons from
incidents, external reviews, and inquests to support improvements in the safety
and quality of care.
The maternity governance and risk team works collaboratively within the risk management framework of the Trust to develop, establish, and maintain risk management systems. Collectively they lead and are responsible for developing robust systems which recognise, report, analyse, evaluate, and minimise the risks to staff, patients, visitors, and the organisation.

To support to

participate in systematic incident investigations, making appropriate use of

capture learning from incidents. Encourage the same systematic approach to
incident investigations throughout the Trust.
Support the delivery of Maternity Incentive
Scheme by ensuring action planning and trackers are maintained, deadlines
achieved and evidence collected, working closely with the senior leadership
team who will have overall responsibility.
Support the delivery of the Saving Babies
Lives care bundle by ensuring teams are working within and towards new
recommendations, highlighting and escalating problem areas and working closely
with the senior leadership team who will have overall responsibility.
Additionally, lead, facilitate and deliver service
improvements within maternity services, providing professional and clinical
advice as necessary.
Support with the implementation of learning
from incidents including regular testing that learning has been effectively
embedded within the team.
Support and contribute to the overall
delivery and development of the risk management agenda as agreed with the
Office of Governance and Corporate Affairs.
Support in producing proportionate incident
investigation reports, written to a high standard and with regard for the
requirements of a diverse audience.
Actively participate in any internal or
external governance and risk groups as required.
Where serious incidents, never events and
other incidents have occurred, support in ensuring effective communication to
all relevant senior managers as required.
Ensure timely and accurate feedback to
clinical areas, providing assurance that processes are in place to monitor
actions for service improvement.
Gather information from within the service,
Trust wide and from external organisations to share learning to minimise risk
and engender a positive and proactive governance culture.
Participate in the gathering of information
and supporting of staff during investigations and coroners inquests.
Support and undertake risk assessments as
required.
Support in the communication with
stakeholders to ensure transparency and support the duty of candour, ensuring
effective communication with regards to the implications of clinical risks and
incidents and appropriate risk management.
