Job responsibilities
Clinical
Develop a high level of clinical decision making,
monitor and improve standards of care, working within their advanced scope of
professional practice.
Contribute towards a multi-professional approach
in the management of patients accessing the in-hospital youth work service.
Support patients and their families and ensure
that they receive the required information to enable them to self-manage, lead,
or participate in their care delivery.
To support, assist and encourage the
multi-disciplinary team in resolving patient problems, by the provision of new
and innovative models of case management.
Support the delivery of adjusted patient
pathways for patients accessing secondary care.
Provide
advice and support on the mental health act to ward staff and support the
receipt and scrutiny of Mental Health Act Paperwork.
Leadership
Be
an innovative, enthusiastic role model providing leadership, guidance, and
advice to staff on operational and professional issues promoting an open and
honest and transparent culture.
Provide
professional visible leadership and a supportive approach that contributes to
creating a climate for achieving excellence in clinical practice, skills and
competencies using the values of the Trust.
In
partnership with the Head of Complex Needs and mental health champion CYP, lead
the implementation of service improvements for young people with Complex presentations
and the evaluation/monitoring of care through audit, complaints monitoring, and
incident monitoring and patient/carer feedback.
Identify, develop, and sustain mechanisms to
support patient involvement and feedback.
Be a resource for health care professionals by
being visible in the clinical area, available, and accessible for support and
advice in relation to the management of patients in the speciality.
Provide leadership to the team and working with
the head of complex needs and mental health champion CYP, develop new roles /
ways of working (High Impact Changes) linked to directorate business plans.
Lead
on consultations and develop strategies for delivering effective care within a
changing environment that improves the quality of care and health outcomes.
Management / Operational
Employ effective decision-making skills to
address complex issues and use effective change management skills to implement
these.
Take
the lead in investigation of incidents / complaints when required, including
detailed reports / letters to complainants.
Work in partnership with the Head of Complex
Needs and mental health champion CYP to manage the team towards achieving
directorate, and Trust objectives.
Identify and develop proposals and business
cases as required for future service innovation.
Manage
and develop and influence the development of practice / services in clinical
areas through the business planning process / service development and in
conjunction with Matrons, Managers and Lead Clinicians.
Delegate work to match capabilities and workload
of team members.
Manage own and other resources effectively to
budget ensuring quality service delivery within budgetary constraints seeking
to reduce budgetary costs by ongoing review.
Monitor sickness, absence, and competence issues
in line with Trust Policy.
Manage recruitment and selection of staff in
line with Trust Policy.
Advocate
Ensure that high standards of clinical care are
given and maintained and act when standards are not being met.
Involve patients and carers in the planning and
delivery of care.
Encourage staff to Value the carer/family
contribution and encourage a partnership of care approach.
Be the patients advocate and ambassador respecting
patient confidentiality and privacy with respect for diverse cultural
backgrounds and requirements.
Lead
the development of a patient focussed education and information giving strategy
within the speciality.
Recognise,
prevent, and diffuse potential areas of conflict and take a key role in the
management of the outcome.
Clinical Governance / Quality
Ensure compliance with policies, procedures and
clinical guidelines for self and others.
Develop and influence policy making and clinical
guidelines internally and externally, necessary to support children and young
people accessing the service.
Continually monitor standards of care and lead
the improvement of care through benchmarking, Nursing Charter Audits and
Clinical Audit.
Support and facilitate research ensure
evidence-based practice in the service.
Disseminate good practice and knowledge by
pursuing opportunities to present work at national conferences or via publication
and encourage team members to do likewise.
Promote patient and public involvement
activities in the service leading to service improvement.
Promote peoples equality, diversity, and rights.
Identify clinical issues and incidents within
the Trust that reduce the quality of care within the service brining any issues
to the attention of the Head of Complex Needs or mental health champion CYP.
Take part in activities to counter these to
ensure the delivery of safe and effective care.
Participates and takes the lead in clinical
audit within the service supporting others to participate and ensuring agreed
results are implemented.
Education / Training
Takes responsibility for own continuing professional
development and performance, including identifying own development needs and
maintain own portfolio in accordance with re-registration requirements.
Participates in the supervision and the teaching
of pre and post registration students, new staff members and junior medical
staff.
To lead and develop with the Head of Complex
Needs and mental health champion CYP on educational programmes in relation to
the speciality.
Supports the clinical area in ensuring the
clinical environment is conducive to effective learning.
Identifies training needs and develop training
resources for nursing staff to enable role development and to meet the needs of
the service.
Take responsibility for providing complex needs
core competencies for untrained and trained staff.
Professional Responsibility
Maintain active status on relevant professional register.
Act always in accordance with your professional
code of conduct and guiding documents, Health Care Standards
Adhere to Trust Policy & Procedure.
Maintain a professional portfolio.
Maintain a high level of awareness of relevant
research issues and trends within the specialist area of practice and in
nursing generally.
Take an active part in the development of service
delivery in the Trust via participation in professional forums and shared
governance initiatives.
Take responsibility for personal development and
education with regard to, Personal Development Plan objectives.
Participate in Clinical Supervision for self and
others and utilise reflective practice techniques.
Provide support and development for staff
through mentorship.
The post holder is responsible for ensuring the
safety of patients at all times and promoting a safety culture through the
effective management of risk.
