Support patient safety in accordance with the patients care plan, and as delegated by members of the multidisciplinary team. To meet the patients identified treatment and care needs as prescribed in the care plan, ensuring high standards of patient centred care are delivered.

To maintain confidentiality, privacy, and dignity in relation to the patients and their treatment at all times.

Deal with patients, on a face-to-face basis, in circumstances involving violent/aggressive behaviour using the application of conflict resolution techniques.

To work effectively as part of the team, liaising closely with all members of the inter-professional team, care staff and other staff as appropriate.

If (or when) required, apply control and restraint techniques as per the Maybo training to fulfil the duties and responsibilities of the role.

To use resources effectively by prioritising daily caseload to maximise the benefit to patients.

To undertake duties as directed by a qualified health care professional.

To undertake frequent therapeutic handling and moving of patients with aids (as needed) and equipment utilising moderate physical effort within clinical and non-clinical settings. This may involve bending, kneeling or frequent repetitive activities often in confined spaces.

Contribute to the clinical governance plan and objectives, maintain an awareness of the NHS modernisation agenda; ensure practice is up to date and that high quality clinical care and support is delivered to patients, relatives and carers promoting independent dignity and choice.

To have basic awareness of the Mental Health Act and the Mental Capacity Act, working to the principles of each Act. and seeking support from specialist advisors in the Trust

To clearly communicate timely, accurate and relevant information regarding patient care with patients, their families and carers, other professionals, and services, to ensure seamless, quality care and to empower patients to become partners in their care.

To maintain standards in organising health information, to safeguard the confidentiality of patient information, be aware and comply with policies, practices and legislation that affect the practice of the Nursing Service. Also, to ensure accurate record keeping, file management and profiling data.

To maintain standards in ongoing health information, both electronic and paper based, ensuring safe keeping and confidentiality accurate record keeping of relevant data and profiling.

To participate and contribute to the development of partnership working; in order to positively influence the development of services, through maintaining an awareness of multi-disciplinary agencies within the community. To ensure effective seamless patient care across partner organisations

To contribute towards the effective and positive working atmosphere of the team and be involved in a 24-hour service.

To monitor and maintain health and security of self and patients in the working area. To identify and report changes that may increase risk to staff, patients, and carers, escalating concerns to the named in charge nurse for the shift

To maintain job competencies through continuous professional development and attendance on relevant courses

To undertake relevant additional training to enhance knowledge and skills and to utilise their Level 3 qualification to fulfil the duties and responsibilities of this role.

To participate in Appraisal process in accordance with the Knowledge and Skills Framework.

To undertake all mandatory / essential training as required by the Organisation.

To adhere to the lone worker policy in order to minimise the potential risks associated with working in the community.

To adhere to Organisational Policies, procedures, and protocols to promote the highest quality of care within the patient experience.

To travel independently between sites on a daily basis, due to the expectation of multi-site working to meet the demand

Critical examine working practices within the department to identify cost improvements and effective measures, develop and implement action plans.

To maintain the record of premium rate activity on the definitive list

To validate all claims for premium rate work against the relevant record in the definitive list

The post holder will ensure the proper use of the Trusts resources such as stationery, telephone usage, photocopying and other consumables in the course of business, ensuring minimal waste and minimal cost.

To maintain the confidentiality of the Trust in respect of patient and staff information obtained at all times, and use such information only as authorised for specific purposes. Report any concerns about the use of such information to the senior manager.

Person Specification

Qualifications

Essential

• NVQ/BTEC Level 3 in Health-related subject.
• Level 2 in Numeracy and Literacy (or equivalent) or willingness to complete within 12 months of employment.
• Willingness to undertake role specific competencies within 12 months.
• Care Certificate or willingness to undertake within 3 months.
• Willingness to complete Maybo restraint training on commencement of employment.

Desirable

• Any further qualifications and training relevant to this post.

Experience

Essential

• Experience of working in a Health and Social Care setting.
• Good interpersonal and communication skills.
• Good time management skills including ability to work to set deadlines.
• Ability to work flexibly, quickly, and accurately.
• Experience of working with patients requiring healthcare.
• Experience of working with people with behaviours that challenge.

Desirable

• Experience with NHS software systems such as PPM or PAS.
• Previous experience relevant to speciality.
