An exciting opportunity has arisen for a flexible, highly motivated Senior physiotherapist with excellent communication and rehabilitation skills to join the Active Together team within cancer and therapy services.
Mid Yorkshire Teaching NHS Trust, Kirklees Active Leisure Trust and Wakefield Councils are working collaboratively with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model to individuals diagnosed with cancer in Wakefield and North Kirklees.
The Active Together programme will run alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare.
We a looking for a dedicated senior physiotherapist with the skills and experience necessary to provide physiotherapy leadership for the active together wakefield service. The suitable candidate will coordinate the physiotherapy and rehabilitation input for patients referred into the service and lead the establishment of evidence-based physical activity rehabilitation pathway for people with a cancer diagnosis across multiple cancer pathways.

Main duties of the job
We are looking for a dedicated senior physiotherapist with the skills and experience necessary to provide physiotherapy leadership for the active together Wakefield service. The suitable candidate will coordinate the physiotherapy, prehabilitation and rehabilitation input for patients referred into the service and lead the establishment of evidence-based physical activity rehabilitation pathway for people with a cancer diagnosis across multiple cancer pathways.
The suitable candidate will be an integral part of the Active Together team working alongside the project lead and the MDT to deliver a high quality effective service.
We are looking for an innovative and forward thinking individual to design and implement clinical protocols for the physical activity and exercise components of the service in collaboration with the wider Active together network. The suitable candidate will be able to act as a professional resource and advisor, providing support and guidance on all matters concerning cancer rehabilitation and prehabilitation including embedding principles of co-production, value-based care, prudent healthcare, supporting self-management and protecting the most vulnerable.

This is a funded 5 year programme which will have staged implementation into all relevant cancer pathways. Patients will receive individual assessments and be triaged into a support pathway suitable to their needs. This may be delivered in Mid Yorkshire Hospitals, selected council and Leisure facilities closest to the patients home or may be a self-management pathway
Key responsibilities

professionally and legally responsible and accountable for all aspects

designated caseload of patients, and organise this effectively and efficiently
with regard to clinical priorities and use of time.

of sources, including those with a complex presentation, using
investigative and analytical skills.

Formulate individualised management and
treatment plans, using clinical reasoning, and utilising a wide range of
advanced treatment skills and options to formulate a specialised programme of
care in conjunction with the patient and carers.
- Plan a care programme for individual patients in
conjunction with other professional staff within the multidisciplinary team
(MDT).
- Provide specialist clinical expertise to
colleagues within the team and referrers to the service.
- Communicate effectively with patients and carers
to maximise rehabilitation potential and ensure understanding of condition. Communication
skills of persuasion, motivation and explanation to encourage patients to
undertake their treatment programme.
- Use verbal and non-verbal communication skills to
gain informed consent with patients where there will often be barriers to
effective communication e.g. loss of hearing, altered perception, expressive or
receptive dysphasia, pain, fear.

have the ability to work within a legal framework with patients who may lack
capacity to consent to treatment.
