Job summary

An opportunity has arisen within the MY Therapy service to recruit a highly motivated Band 5 Occupational Therapist. MY Therapy service is a large and very busy MDT incorporating Occupational Therapists, Physiotherapists and support staff. We are proud to have played a large part in the Connecting Care Closer to Home agenda for Wakefield. Our team is flexible and the successful candidate will be expected to work across a variety of bases across the Wakefield District.

NHS and Council workers, together with the voluntary sector in Wakefield District, have been part of this scheme to pioneer integrated care between a number of organisations in particular Health, Social Care and the voluntary sector to deliver more joined up services closer to home where possible for citizens. The aim is to help people to remain more independent for longer whilst receiving health and social care in their own home where possible.

Adult Community Services received a 'Good' rating when last inspected by the CQC and the successful candidate will instil the Trusts values and behaviours in ensuring quality services continue to be delivered and to strive for further improvement.

Main duties of the job

The successful candidate will work together with colleagues, partners, patients and their families/carers in providing rehabilitation in the community thereby preventing unnecessary admissions to hospital / residential settings whilst also facilitating discharge.

MY Therapy also provide therapy to the Wakefield Intermediate Care unit providing rehabilitation to patients prior to discharge into the community. We also provide an urgent community response service and specialist neuro, cardiac and respiratory and long covid services to patients in the community. We run an internal rotation in the team with opportunities to gain experience in these specialist areas.

To be professionally and legally responsible and accountable for all aspects of own practice, including the management of patients in your care.

To understand and apply the Occupational Therapy process in order to effectively self manage a designated caseload within community rehabilitation, addressing occupational performance and skill deficits, enabling the patient in components of self care, productivity and leisure.

Ensure a high standard of clinical care for the patients under your management.

Accept sole clinical responsibility for a designated caseload of patients, and organise this effectively and efficiently with regard to clinical priorities and use of time.

To be able to use research, clinical reasoning and problem solving skills including activity analysis to identify the therapeutic benefit of client centred interventions and make professional judgements with individuals with diverse or complex needs, or multiple problems.

To be able to formulate specific and appropriate management plans including the setting of time scales and engage individual and/or group participation in the Occupational Therapy treatment programme.

To plan, facilitate, implement and evaluate a range of group interventions according to patient need.

Be responsible for patient carried goal setting, evaluating and modifying these goals appropriately in order for Health Care Support Workers to fulfil their daily duties, in order to follow a rehabilitation programme as determined by the Occupational Therapist.

To be aware of and act appropriately with regards to own and others personal safety when working in community setting, including the ability to effectively deal with and de-escalate confrontational situations.

To utilise a range of standardised and non standardised non Occupational Therapy assessment and techniques.

To assess for, prescribe and evaluate the provision of specialist Occupational Therapy equipment, following a holistic assessment.

To provide a rapid specialist Occupational Therapy assessment and comprehensive risk assessment and using a high level of clinical reasoning skills, formulate an appropriate care pathway to prevent unnecessary hospital admission or facilitate safe and timely discharge.
