Job summary

The post holder will support the Clinical Coding and Data Quality functions by contributing to the accurate, timely and comprehensive capture of patient information across the Trust. Highquality data is essential for effective clinical decision making, operational management, statutory reporting and national NHS submissions.

Working across the Clinical Coding and Data Quality teams, the post holder will assist in maintaining a continuous flow of case notes and clinical information into the Coding Department, ensuring coders have timely access to complete and accurate records. This includes the collection, tracking and distribution of medical records, locating and retrieving notes using the Trusts electronic and paper systems, and liaising with clinical areas to resolve missing or incomplete documentation.

The post holder will also support the improvement of patient demographic, administrative and outpatient procedure data on the Patient Administration System (PAS). This includes investigating and correcting data errors, following departmental SOPs, maintaining data quality standards, and using appropriate information sources to validate and update patient details, clinic activity and procedural information.

Main duties of the job

  • Routinely run PAS reports via report manager to identify outstanding un-coded episodes and distribute daily via an excel spreadsheet to the full coding team.
  • Assist in the daily collection and ensure the timely tracking/sending of case notes into, out of and across Mid Yorkshire Hospital Trust sites to ensure the efficient flow of data within the department, communicating any issues/delays to the Clinical Coding Assistant Team Leader.
  • Efficiently identify the speciality and discharge date of an uncoded inpatient spell using the Trusts Patient Administration System (PAS) to label, file and store case notes accordingly until required.
  • Support Clinical Coding staff by monitoring un-coded lists, rotating case notes within the office, ordering case notes from the medical records library/wards as/when required for coding deadlines and returning completed case notes back to the medical records library.
  • Assist in the receipt, tracking, monitoring and distribution of case notes from the bereavement office and LLP activity within the department.
  • Support the clinical coding process by utilising the Trusts digital systems to undertake basic data entry duties and speciality service lists via the Trusts PAS/Medicode system.
