Job summary
Following a re-structure, an exciting opportunity has become available for a Band 5 Registered Nurse to join PERT (Pinderfields Emergency Respiratory Team).
We are team of Respiratory Specialist Nurses, Advanced Clinical Practitioners, Respiratory Physiotherapists, Advanced Clinical Support Workers & Administrative Co-Ordinators. The service operates 7 days per week including Bank Holidays and provides service cover from 8am until 8pm.
The team is responsible for the Respiratory Virtual Ward and Home Ventilation Service for Mid Yorkshire Teaching Trust.
As part of the role of the Registered Nurse you will be expected to provide in-patient and community reviews so would require a driving licence or access to a vehicle for work.
The team is based primarily at Pinderfields Hospital with cross-site working as required and the Respiratory Virtual Ward covers both the Wakefield and North Kirklees district.
Previous experience within Respiratory is desirable but not essential, however a genuine interest in respiratory is essential
Main duties of the job
- You will work alongside the Clinical Nurse Specialists to complete COPD CQUIN care bundles and Asthma care bundles
- You will undertake home visits as required to patients who are receiving an episode of care under the Virtual Ward, maintaining a safe alternative to in-patient hospital care.
- You will support the TOSCA service and provide home visits to patients requiring overnight sleep study.
- Under supervision of the Respiratory CNS, you will review patients who are on Domiciliary NIV.
- You will support the NIV consultant led clinic
- You will support the Pulmonary Rehabilitation service by providing patient education sessions.
- You will ensure all documentation and communication is maintained to a high standard.
- You will liaise with members of the multi-disciplinary team and refer to other services and health care professionals where appropriate ensuring holistic care.
