Job summary
The post holder will be responsible for the planning and delivery of patient care within Adult Community Nursing Teams. The post holder is responsible for maintaining and delivering high standards of care. These services will be provided in accordance with the Trusts declared values and principles ensuring the delivery of quality, patient centred care.
Main duties of the job
- Clearly communicate timely, accurate and relevant information regarding patient care with patients, their families and carers, other professionals, and services to ensure seamless, appropriate timely care and where appropriate empower patients to become partners and maintain their independence.
- Contribute to the development of strategies, guidance, and protocols in accordance with MY clinical governance systems and objectives.
- To communicate collaborative partnership working with services within the Trust, primary care, and social services.
- To take the lead in team developments as designated by their line manager.
- To perform risk assessments ensuring a safe environment within patients homes and work areas.
- Ensures adherence to organisational Health and Safety, risk, and governance arrangements.
- To maintain standards in ongoing health information, both electronic and paper based, ensuring safe keeping and confidentiality, accurate record keeping of relevant data and profiling.
- To input clinical information onto System One as per MY policy.
- To maintain confidentiality, privacy, and dignity in relation to the patients and their treatment at all times.
- To undertake all mandatory / essential training as required by Mid Yorkshire Teaching NHS Trust.
- To be responsible for maintaining professional registration and continuing professional development.
- Demonstrate a professional approach to work in accordance with the NMC Code of Conduct.
