Job summary
An exciting and unique opportunity has arisen, for 2 Band 6 Senior Physiotherapist within the Active Together service at Mid Yorkshire Hospitals NHS Trust.
1 full time (37.5 hours a week) and 1 part time (22.5 hours a week).
Mid Yorkshire Teaching NHS Trust, Kirklees Active Leisure Trust and Wakefield Councils are working collaboratively with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model to individuals diagnosed with cancer in Wakefield and North Kirklees.
The Active Together programme works with patients undergoing to Cancer treatments and runs alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare.
At Mid Yorkshire we are currently working with patients form the following tumour groups colorectal, lung, head & neck, upper gastrointestinal and haematology, with expansion to further tumour groups planned.
The Post holder will work alongside the other therapy staff, completing holistic patient centred assessments, see patients in 1:1 exercise sessions and in class environments.
There will be close working links with other MDT members within the Active Together team. Good communication skills with patients / carers and the multi-disciplinary team are an integral part of the role.
Main duties of the job
Patient Assessment
The ability to interview and examine a patient incorporating subjective and objective elements in order to identify problems to be treated and/or goals of treatment.
Clinical reasoning
The ability to analyse clinical findings by applying appropriate theory and principles and synthesising them with the assessment findings in order to justify the treatment approach.
Apply a high level of theory and advanced principles relating them to assessment findings in complex cases.
To be aware of own capabilities. Can analyse most complex problems.
Identify problems/goal setting
The ability to define a patients problems and set prioritised mutually agreed timely goals.
Apply a high level of theory and advanced principles relating them to assessment findings in complex causes.
Can assess complex patients.
Takes a holistic view using a breadth of clinical knowledge.
Application of treatment modalities
The safe and effective application of high level physiotherapeutic skills to achieve specific outcomes.
Select and apply a broad range of modalities efficiently and effectively.
Justify selection of modality Progress management plan and achieve discharge
The ability to progress the management plan towards achieving the agreed goals and ultimately discharge the patient from Physiotherapy.
Use reflective practice to improve patient care. Able to progress treatment with most patients.
