Job summary

We are recruiting to our Community based Integrated Care team (ICT) in Wakefield.

ICT is a District-wide service that provides care closer to home, making a difference to our patients and the lives of their carers and relatives. This includes hospital prevention, rehabilitation, out of hours community nursing visits and palliative care at home.

Our Healthcare Support Workers are at the heart of our multidisciplinary teams who support our patients by assisting the Registered Nurses in planning, delivering and evaluating care.

As Healthcare support worker, you will play a vital role in supporting patients within their own homes and community settings. Working alongside registered professionals, you will help provide compassionate, person-centred care that promotes independence and wellbeing.

This will require you to:

  • be able to communicate effectively with a broad range of individuals, both verbally and in writing;
  • share our commitment to maintaining high standards in everything we do;
  • demonstrate that you can work effectively within a team that behaves in accordance with our values.

The roles are varied and busy as you will be working a variety of shifts including long days, nights, weekends and bank holidays.

To work in the ICT, it is essential to hold a Full UK driving licence and have access to a car with business insurance.

All expenses for fuel will be reimbursed.

Main duties of the job

No previous experience in a health care setting is required.

It is essential that within your application you are able to demonstrate an understanding of the role and responsibilities, able to prove how you develop and maintain positive working relationships, along with evidencing how you would demonstrate our Trust values of respect, improving, caring, and high standards.

The minimum hours available for this role is 23 hours per week but full time hours are also available.

You must be able to commit to the standard shift pattern for this role which includes long days, nights, weekend and bank holidays.
