Job summary
The primary aim of the role is to provide effective patient centred care by performing individual assessment of care needs, developing, implementing and evaluating programmes of care that meet the needs of the patient and their families/carers.
The role is multidimensional and requires the post holder to integrate and fulfil all the identified dimensions. As a registered nurse all activities provide opportunities for acting as a role model for staff. Following their preceptorship period the registered nurse is expected to carry out all relevant forms of care without direct supervision.
Main duties of the job
Based at Pontefract Hospital - Wakefield Intermediate Care Unit provides Rehabilitation to the residents of Wakefield.
1. Comprehensive Assessment Evaluate physical, cognitive, and emotional status. Monitor vital signs, mobility, pain levels, and progress toward rehabilitation goals. Identify risks such as falls, pressure ulcers, or complications.
2. Care Planning Develop individualized care plans in collaboration with the multidisciplinary team (physiotherapists, occupational therapists, speech therapists, doctors). Set realistic short-term and long-term goals for recovery.
3. Medication Administration & Symptom Management Administer prescribed medications accurately. Manage pain and monitor for side effects. Support patients with conditions like stroke, injuries, or neurological disorders.
4. Mobility & Functional Support Assist with transfers, ambulation, and exercises as recommended by physiotherapists. Encourage independence in activities of daily living (ADLs) such as dressing, eating, and toileting.
5. Education & Emotional Support Teach patients and families about rehabilitation techniques, medication, and lifestyle changes. Provide psychological support to help cope with limitations and motivate progress.
6. Safety & Risk Prevention Implement fall prevention strategies. Ensure safe use of assistive devices (wheelchairs, walkers). Monitor skin integrity and prevent pressure injuries.
