Job summary
We are seeking to recruit a highly motivated Pharmacy Technician to join our well-established medicine optimisation team. Our team consists of Pharmacy Technicians and Pharmacy Support Workers (PSWs), all working together to ensure the safe provision of medicines across the trust whilst we continue to provide the best service to the patients. The position will involves- Working in close collaboration with the pharmacy team and the broader multidisciplinary team (MDT) to optimise the care and medications of patients. Support and develop other members of the pharmacy team. Take part in training and initiatives aimed at improving services. Ensure continuity of patient care regarding medication upon hospital admission to facilitate the efficient reconciliation of medications within the Trust and the prompt provision of medication upon discharge. Ensure the delivery of a pharmacy service that is patient-focused, proactive, and responsive to the wards by providing support, providing, and developing the service
Main duties of the job
As a member of the Pharmacy Medicines Optimisation team, you will be responsible for providing a comprehensive, integrated ward-based pharmacy service, Undertake meticulous medicine reconciliation reviews on all recently admitted inpatients. Assess suitability of patient's own drugs for use whilst in the hospital environment. Provide specialist counselling for patients and carers about their medications and treatment. To participate in the discharge planning of patients with ward staff and other healthcare professionals both in secondary and primary care to ensure a safe, efficient and timely discharge from our care. Support the training and development of Pharmacy Support Workers, Trainee Technicians, Technicians and Trainee Pharmacists. The Pharmacy operates a 7 day service and as part of this role you will be expected to participate in the evening and weekend working rotas.
