Colorectal Clinical Nurse Specialist

Job summary

The post will be a 12 month secondment 28.88 hours per week The post holder will organise and manage a caseload for a defined group of patients / service users, providing specialist nursing support, advice and education and care to that specific group. They will carry continuing responsibility for the assessment of needs, development, implementation and evaluation of programmes of care and the setting of standards within the defined service group.

Main duties of the job

Undertake nurse-led colorectal follow-up clinics as outlined in the Stratified Risk Guidelines. The aim of these guidelines is to ensure all patients following a colorectal cancer resection are enlisted into an appropriate risk stratified surveillance pathway. Will have continuous delegated responsibility for a defined caseload of patients and will provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers. Perform specialist assessment of patients nursing needs, including Holistic Needs Assessment (HNA), plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills. Organise, prioritise and evaluates workload based on accepted referral criteria in line with operational protocols. Will provide clinical and social psychological support to their patient group using advanced communication skills, education and health promotion to enable them to reach an optimum level of independence of health. To provide specialist advice, supervision and support to their defined caseload and peers. attend and participate in the Multi disciplinary meeting Review patients post surgery on the wards Telephone follow up of patients Joint consultations with colorectal consultants
