Job summary
We are seeking a motivated and compassionate Band 6 Clinical Nurse Specialist to join our paediatric diabetes team. This is an exciting opportunity to provide expert nursing care and support to young people aged 16-19 years living with diabetes, working collaboratively with a multidisciplinary team to improve patient outcomes and promote holistic care. We welcome applications from experienced diabetes specialists from both adult and paediatric nursing backgrounds. The post holder will deliver specialist diabetes nursing care, providing expert clinical support and education to children and adolescents up to 19 years old, as well as their families and carers, within the Wakefield district. Care will be delivered in line with agreed policies and protocols, underpinned by the latest research and evidence-based practice, and aligned with both local and national standards. The successful candidate will support 16-19 year olds through their transition to adult services. The post holder will play a key role in empowering children and young people with diabetes to actively engage in their own care, fostering collaborative partnerships with healthcare professionals. They will support patients in setting realistic and achievable personal goals to enhance self-management and wellbeing.
Main duties of the job
Will have continuous delegated responsibility for a caseload of patients up to the age of 19 with type 1, 2 or other forms of diabetes. They will provide an effective and patient focused service which is considerate on needs of users and providers. Organises, prioritises and evaluates workload based on accepted referral criteria and discharge criteria in line with operational protocols. Will provide clinical, social and psychological support to their patients using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health. Will undertake specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills. To provide specialist advice (automated insulin delivery, multiple daily injections, variable doses and insulin adjustments, interpretation of continuous blood glucose monitoring, carbohydrate counting), supervision and support to their defined caseload and peers, in a variety of settings (hospital, home, school, via telephone or face to face). Responsible for providing evidence based practice. Will need to participate and liaise with other professionals regarding safeguarding children and young people including referrals to social care and attendance of child in need meetings and child protection conferences
