Job summary

The Integrated Transfer of Care Hub are looking to recruit Discharge Co-ordinators (previous applicants need not to apply) to work on the wards across the acute hospital to increase patient flow and optimise patient experience surrounding all transfers of care. We are looking for energetic and caring people who have an understanding and some knowledge of the health and social care environment to coordinate discharges and work within the multi-disciplinary team. This is a seven day service that provides single point of contact across all wards and hospital sites within Mid Yorkshire Hospitals Trust for staff, patients, families, carers and associated people and co-ordinate / contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues. Please note that this role is not eligible for sponsorship under the Skilled Worker route

Main duties of the job

To promote the correct implementation of discharge processes throughout Mid Yorkshire hospitals to ensure safe and timely discharge and smooth transfer of patients. Actively participate in the ward based discharge activity (i.e. SAFER, EDDs, board rounds and the identification of patients for transfers to PMU and DDH) to ensure that discharge activity is occurring for every patient within the ward. To maintain momentum of discharge planning throughout the entire process. Actively work with the discharge team and hub to ensure patients are identified for transfer to Gate 11 discharge hub to ensure acute beds are available as soon as possible. Provide concise and detailed information/handover regarding patients discharge plans. Work closely with all internal and external partners and agencies to ensure a seamless service, including working to reduce delayed discharges. To link closely with clinical Site managers to ensure effective use of beds. To promote best practice in achieving high quality discharge planning To use tact and diplomacy where there is dispute over complex issues regarding leaving hospital and there may be disputes regarding courses of action to be taken. To liaise with relevant staff and agencies on a daily basis to resolve daily operational issues and ensure discharges are managed effectively. To support the Discharge Sisters and hospital staff in managing difficult discharges..
