As a Clinical Team Leader in the community, you will:
- Provide visible leadership and line management for the Integrated Care Team (ICT), supporting other teams when required.
- Offer clinical expertise and advice in the management of complex cases, palliative care, and safeguarding, ensuring high standards of care at home.
- Ensure all team members are clinically competent, well-supported, and working within professional guidelines.
- Maintain oversight of record-keeping standards, including medication charts and documentation audits, ensuring compliance with NMC and organisational policies.
- Work closely with the Hospital Discharge Team and wider system partners to support safe, timely discharges and reduce avoidable hospital admissions.
- Promote and embed clinical supervision, staff development, and evidence-based practice across the team.
- Support and lead service development, using audits and performance data to drive improvements and meet local and national targets.
- Manage complaints, incidents, risks, and staff concerns, escalating appropriately and ensuring lessons are learned.
