As a Clinical Team Leader in the community, you will:

Provide visible leadership and line management for the Integrated Care Team (ICT), supporting other teams when required.
Offer clinical expertise and advice in the management of complex cases, palliative care, and safeguarding, ensuring high standards of care at home.
Ensure all team members are clinically competent, well-supported, and working within professional guidelines.
Maintain oversight of record-keeping standards, including medication charts and documentation audits, ensuring compliance with NMC and organisational policies.
Work closely with the Hospital Discharge Team and wider system partners to support safe, timely discharges and reduce avoidable hospital admissions.
Promote and embed clinical supervision, staff development, and evidence-based practice across the team.
Support and lead service development, using audits and performance data to drive improvements and meet local and national targets.
Manage complaints, incidents, risks, and staff concerns, escalating appropriately and ensuring lessons are learned.
