Job summary
Are you an enthusiastic individual who has a keen interest in all aspects of skin health and integrity. This could be an opportunity to develop your sills in this field or be an existing tissue viability nurse who would like to join out existing tissue viability team. You will have the opportunity to work across sites and throughout the MYTT wider community. You should be able: -to provide clinical, social and psychological support to their patients using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health. -to provide specialist advice, supervision and support to their defined caseload and peers. -to provide evidence based practice. -to support the Matron for Tissue Viability in leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement. -to work collaboratively with the Tissue Viability Team and Allied Health Professionals, with in their own scope of practice. -to ensure cost effective use of all resources. -to promote programmes of health education and health promotion relevant to service. -to work within national policy frameworks relevant to the service. -to deal effectively and professionally with patient / service.
Main duties of the job
You will working within a Trustwide Tissue Viability Team and: -provide advice and intervention to all patient groups with tissue viability needs that are referred into the service. -take part in triaging referrals and arranging appointments. -contribute to the development of the tissue viability service. -be responsible for the provision of advice and support to Trust and non-Trust healthcare professionals in relation to Tissue Viability. -provide education and training on Tissue Viability, including wound management, pressure ulcer prevention, pressure relieving equipment and leg ulcer management. -discuss/demonstrate dressing choices/techniques for wounds at request of referring professional. -liaise with acute Trust, community district nurses, residential homes, nursing homes, and other disciplines regarding the management, discharge and transfer of patients with Tissue Viability requirements. -contribute to achieving the objectives of the Trust by implementing quality improvement strategies in relation to pressure ulcer prevention to reduce trust acquired patient harm. -take an active part in the development of nursing in the Trust via participation in professional nursing forum and shared governance initiatives.
