Job summary
The primary aim of the role is to ensure that a high quality service is delivered to patients with Haematological conditions in a timely manner. The role is multidimensional and requires the post holder to integrate and fulfil all the identified dimensions. As an Advanced Clinical Practitioner, all activities provide opportunities for acting as a role model for staff. The Advanced Clinical Practitioner will act as an autonomous, accountable practitioner working on their own initiative and in collaboration with nursing, medical and allied health professionals to assess individual patient needs, initiate investigations, determine a differential diagnosis and initiate appropriate holistic evidence based treatment and care. The post requires working with inpatients and out patients. The post holder will run clinics comprising of patient on simple and complex chemotherapy reviews and relapsing patients requiring further investigations. They will work on Gate 21 (18 bedded Haematology ward) and complete the ward round as well as presenting patients caseloads at MDT. Patients also require review and plans developing on outlying wards at Pinderfields Hospital. The post holder is required to review patients attending Acute Oncology day unit with Haematological conditions and support and advise staff through the emergency helpline.
Main duties of the job
The Advanced Clinical Practitioner will: Manage their own caseload of patients who present with undifferentiated and undiagnosed illness. Undertake research and audit within the clinical setting to develop an evidence base. Provide formal and informal teaching, support and clinical supervision to nursing staff and junior medical staff. Provide support to more junior members of the advanced clinical practitioner team and the wider clinical teams. Ensure maintenance of clinical excellence. Support the wider multidisciplinary team, undertaking independent comprehensive assessment and management of patients to incorporate physiological, psychological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes, including, where appropriate, the discharging patients from the service when treatment is complete/optimum outcome is achieved.
