Job summary

The Band 6 CAIS Nurse (Community Learning Disability Services) with RDaSH provides specialist nursing leadership and clinical expertise to adults with learning disabilities who present with complex needs, including behaviours that challenge and co-existing mental or physical health conditions. The role focuses on intensive, person-centred community interventions to prevent hospital admission, reduce crisis escalation and promote positive outcomes in line with the Transforming Care agenda.

Main duties of the job

  • Deliver high-quality, evidence-based nursing assessments, care planning and interventions within community settings.
  • Provide intensive, short-term support during periods of crisis to reduce risk and avoid inpatient admission.
  • Conduct comprehensive risk assessments and implement positive behaviour support and trauma-informed care approaches.
  • Act as a care coordinator and key professional within the MDT, liaising with families, carers, social care and partner agencies.
  • Lead on clinical decision-making, supervise support staff, and contribute to teaching and mentoring.
  • Monitor, review and evaluate care outcomes, ensuring care is responsive and recovery-focused.
  • Maintain accurate clinical records and ensure practice complies with RDaSH policies, safeguarding, Mental Capacity Act and professional standards.
  • Contribute to service development, audit and quality improvement initiatives.
