Community Cardiac Healthcare Assistant

Job summary

An exciting opportunity has arisen for a motivated individual to join our Community CardiacService within the Rotherham Doncaster and South Humber NHS Foundation Trust(RDASH).The postholder will undertake delegated clinical tasks, support patient monitoring andcontribute to reducing avoidable hospital admissions in line with RDASH trust policies,procedures and values, working under the supervision of a registered nurse to support thedelivery of safe, effective and compassionate cardiac care in the community, running clinicsand home visits to support patients

Main duties of the job

The main duties of the rule will include:

- Carrying out heart failure clinic reviews

- Undertaking routine clinical observations including blood pressure, pulse, oxygen saturations, temperature following RDASH protocols.

- Carrying out venepuncture.

- Carrying out ECG recordings where trained and assessed as competent.

- Supporting patients with long term cardiac conditions, including heart failure, arrhythmia

and coronary heart disease.

- Independently managing 24-hour tapes including the configuring and downloading of data.

- Application of 24-hour heart monitors in a clinic/home base setting and utilising a self-fitoption.

-Accurately report findings and any changes or concerns promptly to senior nurses.

-Monitoring of waiting lists to maintain a 4-week response time liaising closely with Adminand Clinical staff.

-Maintaining confidentiality in accordance with Trust policies and national guidelines.
