Job summary
- The Healthcare Assistant will contribute to the assessment, implementation, and delivery of high-quality patient care within the Specialist Continence team
- Will demonstrate responsibility to patients, relatives, and colleagues through contribution to the development and monitoring of agreed evidence-based practice and to the health and social care programmes
- To provide a comprehensive call answering and administration support service for the continence prescription service
- To liaise with patients regarding their continence product needs, generate prescriptions, and provide correspondence to other health service professionals
- Follows a plan of care as agreed by the relevant professional
- Providing the relevant care package to the patient / service user
- Reports back to senior staff all findings and evaluations
- To provide administration and telephone support to the Doncaster Continence Prescription Service.
- The Doncaster Prescription Service will prescribe continence equipment to all patients registered with a Doncaster GP
Main duties of the job
Undertake agreed activities as delegated by a registered nurse, with patients / service users under the direct /In-direct supervision of the qualified staff.
- Ensure that access to files in the computer is password controlled to maintain security.
- Deal with all enquiries in a courteous and efficient manner, recording messages as per agreed systems and escalating as per protocols.
- Document all call information according to agreed protocols, ensuring all appropriate information is captured on the patient record accordingly.
- Set care plans and coordinate appointment/home visits for the Specialist Continence Service.
- Contributes to the assessment, implementation, and delivery of individualised evidence-based care.
- To maintain accurate records in the patient's own home or electronically, record new problems as they are identified and report all actions to nurse in charge/case manager.
- Provide feedback of current patient / service user status to appropriate members of the-professional team.
- Develop a rapport with patients / service users and encourage them to participate in the rehabilitation/ healthcare programmes and activities.
- Assist the relevant professional during assessment / treatment / intervention where required and assist in educational and health promotional activities.
