The Long-Term Conditions pathway, part of the Children's Care Group within RDaSH, are seeking to recruit an enthusiastic and motivated Health Care Support Worker to join our Children's Community Nursing Service. The service has a clear focus on meeting the needs of children, young people and families through health support and intervention in the community setting.

This is a fixed term position for 12 months, (potential to become permanent), 30 hours per week.

You will be part of an integrated team working alongside Children's Specialisms (Children's Continence Nursing, Epilepsy, Special School Nursing, and TB). The service provides nursing intervention, support and advice to children, young people and families across the Doncaster Borough.

Through clinical competency assessment, training and shadowing experience the post holder will become competent in the delivery of a wide range of clinical interventions. The post holder will hold responsibility for the management of medical equipment; including ordering, servicing, cleaning and the coordination of delivering and collecting to/from family homes and other settings. The health care support worker will be able to work independently and assist in carrying a caseload under the supervision of senior nurse, completing tasks according to need.

Main duties of the job

The Post holder will support and work under the guidance of the Children's Community Nurses to deliver personalised plans of care and health interventions. These support children and young people to have their health needs met in the community setting, facilitating early discharge and reducing hospital admissions.

The post holder will contribute to the planning and direct delivery of care as well as working with a wide MDT to ensure the continuity of care supporting transition, safeguarding requirements, safe discharge from hospital and escalation of needs.

The post holder will need excellent communication skills to build close relationships with children, young people and families offering them encouragement and support.
