Job summary

This is a full time position 37.5 hours per week based in Doncaster.

The Doncaster 0-5 Health Visiting Service is an outstanding forward thinking Team who are proud to have recently won the RDaSH Clinical Team of the year award 2020 and are rated as Outstanding by CQC. We pride ourselves in leading the way with care and offering our Health Visiting Team an enhanced training package of tools to support their exemplary practice. Health Visitors have the opportunity to complete evidence based training including Brazlton (New Born Observation), Promotional Guides and the Solihull Approach is embedded into our everyday practice.

We have positive partnerships with Public Health, Midwifery and local Council partners and are involved with joint innovative practice championing the First 1001 Critical Days. We have close links with the Institute of Health Visiting, Sheffield and York Universities with which we are developing research and piloting further support tools for practice.

Applications are welcomed from SCPHN -Health Visitor NMC Registered Practitioners who are proactive and enthusiastic with excellent communication, interpersonal and organisational skills and are keen to work in Doncaster.

Main duties of the job

As a Specialist Community Public Health Nurse (SCPHN) you will work as part of an integrated team contributing to family health needs assessment. The post holder will lead the 0-5 Healthy Child Programme and work in partnership with the family, education and other agencies to develop implement and evaluate packages of care. Our service delivery model reflects the National 4, 5, 6 model with a focus on the 6 High Impact areas.

We deliver services in pathways at Community, Universal/Universal Plus and Universal Partnership Plus levels. This is underpinned by a Single Point of Contact (SPOC) which supports our practice by responding to unplanned care enabling the Universal team to focus on providing quality core Health Visiting care. Successful applicants will be allocated to work in one of 4 areas - North, South, East or Central area of Doncaster.
