Job summary
JOB PURPOSE
The Physiotherapist will act as a named professional in the assessment, planning implementation and evaluation of rehabilitation care for patients, in community and inpatients. This will be achieved by following evidence based, individualised plans of care.
SCOPE AND RANGE
Rotational post across Doncaster and Rotherham including Mental Health,Learning Disabilities, Home First Services and Neuro Services.
Rotations will be for 6 months in each service.
Main duties of the job
Clinical
Conduct comprehensive assessments and deliver evidence-based physiotherapy treatments for diverse conditions.
Plan, implement, and adapt interventions collaboratively with patients to ensure effectiveness and progress.
Provide person-centred care and rehabilitation across various settings.
Work autonomously, managing and prioritising own caseload efficiently.
Collaborate with the multidisciplinary team to deliver coordinated, timely rehabilitation.
Communicate effectively with patients, carers, and professionals, using empathy and clarity.
Refer to other professionals/agencies as appropriate and act as a patient advocate.
Maintain dignity, privacy, and holistic well-being for patients and carers.
Participate in weekend rota when required.
Management & Leadership
Supervise and support Rehabilitation/Physiotherapy Assistants in their development.
Represent Physiotherapy in MDT meetings and service development forums.
Support implementation and compliance with Trust policies.
Clinical Governance
Adhere to CSP, HCPC, and all relevant standards, codes, and policies.
Contribute to quality improvement initiatives and safe equipment management.
Comply with Health & Safety and Infection Control protocols, including risk assessments.
