Mental Health Nurse; Health & Wellbeing Team

Job summary

To provide high quality physical and mental health assessment and intervention for those patients who are on an extended recovery journey with secondary mental health services.

To provide treatment, including depot administration, blood taking and monitoring service to all patients in Rotherham who are currently prescribed Clozapine and Lithium therapies.

There is an expectation that the post-holder will work collaboratively with the multi -professional team, to meet the needs of patients and to develop and maintain, close and professional working relationships with colleagues across other statutory and voluntary agencies.

Main duties of the job

To work as part of the Health & Wellbeing team, delivering continuous, effective, evidence based care.

Working in clozapine clinic.

Planning and delivering care, including administration of depot medication.

Physical Health Monitoring in alignment with best practice for SMI including ECGs, venepuncture and physical health checks.

To liaise effectively with all members of the MDT and other agencies involved in the care of the individual, thereby maintaining and enhancing close professional working relationships.

To carry out risk assessment and implement appropriate interventions/management plans in accordance with Trust policy.

To assess in accordance with local Safeguarding Adults and Safeguarding Children Policy.

Ensure that users and carers views are appropriately sought and incorporated in the planning and delivery of services.

Promote positive working relationships between all agencies within the locality and Trust.

Provide evidence-based interventions within National and local protocols and ensure that review of the effectiveness of interventions/treatment takes place.

To work within the multi-disciplinary team to ensure clients/ service users receive a comprehensive package of care.

Support patients to adopt health promotions and wellness recovery, utilising social prescribing where appropriate.
