The Primary care routine pathway within the Complex Emotional Care Mental Health Team has an exciting opportunity for an experienced adult mental health nurse to join our team.

This role will involve offering triage, assessments, reviews and interventions for patients referred to the team, in accessible locations across North Lincolnshire. Networking between Primary Care and Secondary Care Services is a central part of the role to ensure the patients journey between services is seamless and care and treatment is provided in a timely manner. The role will also involve working with the wider team to triage and offer routine mental health assessments to adults experiencing mental health problems within Primary Care.
Main duties of the job
As a member of the Team the post holder will provide high quality nursing assessment and interventions within the primary care routine Pathway.
The post holder will be an enthusiastic, highly organised and motivated mental health professional who can provide high quality nursing expertise and support to patients.
It is essential that the applicant can demonstrate post registration experience as an RMN, which may include inpatient and community settings.
The following tasks and duties will also form part of the role, however this list is not exhaustive:
Triage of incoming referrals to establish urgency / risk
Holistic mental health assessment
Development of appropriate treatment plans / signposting to relevant third sector organisations
Recruitment, supervision, and deployment of staff.
Advice, training and induction and mentorship of learners.
Audit, research and service monitoring.
Ordering, storage, monitoring and administration of medication.
Liaison within the multi-disciplinary care team, including carers, voluntary agencies etc.
