Job summary
The CRHT is a multidisciplinary (MDT) team that provide 24/7 urgent clinical triage and advice via the crisis telephone line, as well as conducting urgent and emergency mental health assessments for people in crisis. In addition to this, the team also provide intensive home treatment to those patients admitted onto the caseload as an alternative to hospital admission. The CRHT also act as gatekeepers for all inpatient admissions, crisis house admissions and facilitate early discharge from the inpatient wards.
To assist in providing a safe environment that is satisfactory to maintain the physical and mental well-being of patients and staff.
To deliver appropriate and effective evidence based clinical care without direct supervision.
To actively support and participate in the provision of joint Health and Social Care Services/interventions, ensuring co-operation and collaboration at all times.
To ensure everyone has their right to be treated with respect and have their values and beliefs maintained and that they receive care that maintains their privacy and dignity
Main duties of the job
To support with leading a team of Support Treatment and Recovery Workers in delivering continuous effective evidence based care.
To carry out comprehensive assessment including risk assessment with patients on admission to the team and at appropriate intervals thereafter.
To devise and implement individual strategies of care in conjunction with the client, care giver and members of the multi-disciplinary team as appropriate, regularly evaluating and modifying the strategy as required.
To provide a high standard of care in accordance with Trust policies and procedures.
To understand the principles of, and contribute to, the facilitation of a consistent and effective approach to the nursing process and care programme approach and complete the documentation to the required standard.
To carry out clinical procedures in relation to effective patient care and contribute to the training of junior staff in these procedures as appropriate.
To ensure the correct administration of drugs in accordance with the Trust's policy and legal requirement.
To actively participate in the Trust observation/engagement policy and be aware of the patient's whereabouts if required, at all times.
