Job summary

An exciting secondment development opportunity has arisen for a motivated and enthusiastic individual to step into the role of Community Psychiatric Nurse within the Central Locality AMH team. This position is only open to candidates currently in a clinical band 6 role or a band 5 wishing to progress in their career within all services.

Central is a busy, dynamic and changing team that implements changes and welcomes positive patient care. The ideal candidate will be a team player, have a passion for community care and working in a diverse manner as well as excellent communication skills. The successful candidate will carry a case load and be responsible for the every day management. Car driving is not essential but must have means to being able to go to appointments independently.

Main duties of the job

Responsibility for the development, organisation, delivery and management of a high quality community mental health service, within the agreed budget and service level agreements.

Responsibility for the delivery of services that meet national, regional, trust wide and local commissioning targets. This will include the management of referrals, ongoing cases, prioritisation, allocation and supervision of work within the Team, ensuring compliance with Care Programme Approach/Care Management standards and Clinical Governance.

Responsibility for establishing and maintaining effective working relationships with other agencies to promote services and develop an integrated approach to care provision for users and carers.

To assist the Locality Manager in ensuring that the Team delivers timely and targeted interventions within the resources allocated, maintaining consistent quality standards and good information systems.
