Job summary
An exciting opportunity has arisen to join our multidisciplinary community team as a Community Sister/ Charge Nurse for the Specialist Continence Service. This includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings.
The Specialist continence Service team covers the whole borough of Doncaster and provide all aspects of continence care to patients in their own homes and in a clinic settings. The service provides specialist continence care for patients with complex health needs who are at risk of deterioration which may result in declining quality of life, avoidable hospital admissions, or an unnecessary length of hospital stay.
The aim of the service is to promote self-care and independence for patients living with continence associated conditions, and to provide support, education for patients and carers.
We are seeking to appoint a Sister/Charge Nurse who can demonstrate the vision and skills to work innovatively, flexibly and in partnership with other agencies supporting the MDT team assessment and provision of care to the high standard we aim for. The Sister/Charge Nurse is responsible for the day-to-day management of the team, as well as clinical decision making.
Main duties of the job
The role requires an autonomous practitioner who is qualified to manage a caseload of complex patients independently. The role requires delivering education and training to patients, carers and colleagues. There is a requirement to facilitate and manage link nurse systems quarterly.
This role involves assessing & giving advice over the telephone and face to face, utilising excellent communication skills, IT skills, clinical expertise, telephonic clinical assessment, diplomacy skills and judgement to ensure that referrals into the service are managed effectively and the relevant services are involved within appropriate timeframes.
The care is delivered in a range of clinical settings across Doncaster as well as in patients own homes. Excellent time management required and ability to manage workload appropriately.
