Job summary
The purpose of the Home First Patient Flow Coordinator is to
- ensure proactive identification of patients who meet the admission criteria for inpatient rehabilitation on TRH site, the signpost patients to more appropriate discharge pathways and in their role to provide advice to support the correct identification and utilisation of discharge pathways available within Home First Services.
- To ensure effective flow management of the inpatient scheduled and unscheduled pathway from point of referral, ensuring appropriate movement and placement throughout the patient's journey.
- To escalating capacity exceptions out with recognised/staffed capacity areas to identified line manager.
- There will be a requirement to work contracted hours over a 7-day period.
Main duties of the job
Key purpose:
- Balancing the demands of scheduled and unscheduled admissions
- Working with RDaSH colleagues and partners across place to identify patients who can be referred into all Home First and wider Physical and Neurodiversity Care Group services.
- Assist with the creation of early capacity and optimise patient placement and management of variance working with ward managers and team leaders across all pathways.
- Ensuring seamless patient journeys.
- Provide leadership and guidance for the flow of patients.
