Job summary

  • The Band 6 Rehabilitation Sister is responsible for providing the individualised care of patients in ward based environment, has day to day accountability for the quality of care delivered in their department.
  • Working in partnership with other professionals and agencies, with patients, families and carers in assessing, planning and implementing an individualised rehabilitation programme specific to the person's needs.
  • Provides nursing expertise, evidence based practice and Clinical Leadership to the Rehabilitation team.
  • Support the organisational developments, delivery of quality care and management of the department through partnership working within the Clinical leadership team.
  • Facilitate learning and development of the rehabilitation team through education.

Main duties of the job

  • Will deliver person centred plans of care e.g. assisting patients with achieving activities of daily living, to ensure Rehabilitation goals are met.
  • Provide clinical nursing expertise to the management of individual needs e.g. Continence, Tissue Viability.
  • Assess the health and health related needs of patients, clients, their families and other carers and identifies and initiates appropriate steps for effective care for individuals and their families.
  • Contribute to the interdisciplinary team in monitoring the patient's progress and ensuring that goals are a true reflection of the individuals needs, taking into consideration their choices and preferences.
  • Work in partnership with core disciplines to plan and implement treatment plans relating to individuals health needs, using skills in clinical assessment, decision making and prescribing.
  • Ensure that wherever Nurse Prescribing is undertaken the nurse prescribing policy is adhered to.
  • Assess diagnose and treat specific conditions in accordance with agreed nursing and medical protocols.
  • Assess and manage critical and clinical events to ensure safe and effective
  • Consider the changing needs of the patient in relation to environment
