Job summary

  • The post holder will support the Clozapine and Lithium Clinic to deliver high-quality patient care, upholding patient dignity delivered with kindness and compassion.
  • The post holder will demonstrate responsibility to patients, relatives, and colleagues through contribution to the development and monitoring of agreed evidence-based practice and to the health and social care programmes.
  • The post holder will provide and monitor the nursing care for a designated group of patients.
  • The post holder will also actively contribute to an effective learning environment.

Main duties of the job

  • Be accountable for providing person centred quality nursing care delivered with kindness and compassion.
  • Act in the best interest of the patient and their families.
  • Maintain professionalism at all times and uphold the Trust values and behaviours.
  • Utilise knowledge and skills in making evidence-based decisions.
  • Any further training identified locally relevant to the post.
  • Works within scope of competence and identify own limitations.
  • Provides high quality, safe and compassionate care under the direct /indirect supervision of the registered nursing staff.
  • Contributes to the assessment, implementation and delivery of individualised evidence-based care.
  • Maintains accurate records in line with Trust and NMC policy.
  • Provide feedback of current patient status to appropriate members of the professional teams involved.
  • Develop a rapport with patients and carers.
  • Work in partnership with patients/carers and the MDT to promote health and independence.
  • Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a patient and their family.
  • Assist the relevant professional during assessment/treatment/intervention where required.
  • Support the patient to make use of available services and facilities.
  • Follow Health and Safety regulations, promote where possible a safe environment.
  • Awareness of and adherence to all Trust policies and procedures.

  • Evidence of continuing personal learning.

  • Venepuncture and ECG skills.
