Job summary

The Healthcare Assistant will contribute to the assessment, implementation and delivery of high quality patient care within the Specialist Continence team. Will demonstrate responsibility to patients, relatives and colleagues through contribution to the development and monitoring of agreed evidence based practice and to the health and social care programmes.

Main duties of the job

  • Undertake agreed activities as delegated by a registered nurse, with patients / service users under the direct /In-direct supervision of the qualified staff.
  • Contributes to the assessment, implementation and delivery of individualised evidence based care.
  • To maintain accurate records in the patient's own home or electronically, record new problems as they are identified and report all actions to nurse in charge/case manager.
  • The post holder is required to take personal responsibility to incorporate the 6 C vision of care into their care delivery
  • Provide feedback of current patient / service user status to appropriate members of the-professional team.
  • Develop a rapport with patients / service users and encourage them to participate in the rehabilitation/ healthcare programmes and activities.
