Job summary

The post holder will work as a member of a multidisciplinary CMHT providing a service to clients experiencing mental health difficulties of a serious and enduring nature.

To be responsible as the lead professional for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers as part of a multidisciplinary approach.

To offer first contact assessments, recognising needs of individuals and signposting to the most appropriate care pathway.

To provide professional advice and support to other agencies and members of the multidisciplinary team. To act as a role model and resource for junior/less experienced staff.

Main duties of the job

  • Responsible for the assessment, planning, implementation and evaluation of evidence based care, including recovery and health promotion for an identified group of patients.
  • Responsible for the appropriate clinical risk assessment of patients contacting the crisis service as appropriate including liaison the MDT.
  • Providing effective advice, guidance and decision making culminating in the coordination of any reviewed plan of care.
  • Responsible for ensuring they understand and meet their professional responsibilities under the child protection legislation.
  • Responsible for ensuring that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented.
  • Responsible for establishing therapeutic relationships with service users, and the implementation of evidence based therapeutic interventions with appropriate boundaries in accordance with professional codes of conduct.
  • Responsible for ensuring that they clearly communicate with clients and carers, actively listen to establish an understanding of needs and is delivered in accordance with the requirements of the Nice guidance and clinical pathways.
  • Responsible for deputising and providing appropriate support and caseload cover for colleagues in the absence of the team leader.
  • Responsible for liaison with staff working within other mental health services, in the community, hospital and other agencies.
  • To promote a positive image of mental health services and care provided.
