Community Midwifery/Outpatients Care Coordinator

Job summary
We are seeking an organised, proactive, and compassionate Care Coordinator to support the smooth running of our Maternity Community and Outpatient services. This role is central to ensuring high-quality, patient-centred care by coordinating midwifery clinics, managing administrative processes, and acting as a key point of contact for women, families, and the wider multidisciplinary team.
As a Care Coordinator, you will play a vital role in supporting the operational flow of community midwifery services, including booking appointments, arranging home visits, processing discharges, and ensuring timely follow-up care. You will work closely with midwives, obstetricians, support staff, and external healthcare partners to maintain efficient clinic and service delivery.
You will be responsible for a range of administrative and coordination tasks such as preparing and processing outcome sheets, managing pick lists, cashing up clinics, ordering stock, and maintaining accurate patient records. The role requires excellent communication skills and the ability to manage rapidly changing priorities, particularly when accommodating last-minute adjustments due to staff availability or urgent patient needs.

Main duties of the job
- Coordinating community and outpatient midwifery clinics and home visits
- Liaising with women, families, and the multidisciplinary team
- Processing discharges and arranging follow-up appointments
- Booking appointments and supporting outpatient clinic administration
- Preparing and updating outcome sheets and managing clinic pick lists
- Supporting financial processes, including cashing up clinics
- Ordering and maintaining stock for clinic areas
- Responding quickly and effectively to staffing changes and service needs
- Ensuring excellent communication and continuity of care throughout the maternity pathway
