Job summary

An exciting opportunity has arisen for anAllied Health Professional (AHP) Duty Coordinator within our Urgent Community Response (UCR) team, supporting both the Virtual Ward and Urgent Care pathways across the Hastings & Rother area.

The Virtual Ward is an integral part of our UCR service, operating seven days a week to provide acute-level care for patients in the place they call home. Our service supports both early discharge from hospital and admission avoidance, ensuring patients receive timely, person-centred care within their own environment. Our multidisciplinary team includes Occupational Therapy, Physiotherapy, Dietetics, and Speech and Language Therapy.

We are seeking a dynamic, motivated, and organised individual to play a key role in:

  • Coordinating and supporting therapy teams
  • Triaging and prioritising referrals
  • Conducting wellbeing calls
  • Supporting urgent therapy visits
  • Assisting with assessment, equipment provision, and rehabilitation for patients with Physiotherapy or Occupational Therapy needs

Main duties of the job

The successful post holder will have previous experience working within clinical settings and will join a highly supportive team that utilises an inter-professional model to deliver care to a broad and varied patient group. Both our EDGH and Conquest teams are committed to ongoing professional development, offering regular clinical supervision and opportunities to enhance your skills.

As a Band 4 AHP Duty Coordinator, you will:

  • Work as part of an established multidisciplinary team while also demonstrating confidence in working autonomously
  • Lead the triage and prioritisation of referrals entering the Therapy teams
  • Support the coordination of patient care, including the allocation of visits and the management of effective on-boarding and off-boarding processes
  • Work flexibly in the community, visiting patients in their own homes
  • Undertake basic assessments and treatment plans, and provide clinical support to therapists managing their caseloads

Due to the community nature of this role, you must hold a valid driving licence and have access to a car to ensure you can visit patients in their homes and travel between sites as required.
