We have an exciting opportunity for a Rehabilitation Support Worker to support the delivery of our rehabilitation strategy for Intermediate Care Ward at East Sussex Healthcare NHS Trust. This role will support our therapy team to provide high quality rehabilitation to patients whilst awaiting discharge from the Acute hospital setting
The ESHT Rehabilitation strategy is focused on improving the quality and performance of our acute and community services to deliver high quality rehabilitation across East Sussex. This is a dynamic and changing environment, and the strategy is working to support our staff, teams and the organisation as a whole to make meaningful changes that bring about improvements in rehabilitation for the benefit of our population. The role of the band 3 Rehabilitation Support Worker is key in delivering our strategy.
Main duties of the job
To support multi-disciplinary professionals in providing a comprehensive rehabilitation programme in order to achieve the highest attainable level of independence for each client within an agreed timescale
To provide individual or group interventions as identified by the health professional assessment.
To hold a case load of non-complex cases under the supervision of a senior team member.

- GCSE grade 4 or GCE C and above
- Level 2 functional skills or health care support worker apprenticeship / NVQ level 2 in relevant subject
- Level 3 apprenticeship for example senior health care support worker or similar and / or NVQ level 3 in relevant subject or equivalent level of knowledge and experience
Experience

- Experience in health or social care settings such as employment, work experience volunteering or placements
Desirable
- Demonstrate the values of the NHS and commitment to personal and professional development
