Main duties of the job

To work alongside stroke therapyteam in both the acute and communityto provide a flexible range of integrated, therapeutic services for the local stroke population; to identify and review individual clients' needs, ensuring that they are receiving an effective, equitable and personalised service. To coordinate timely client care by supporting and encouraging integrated working across multiple services and organisations, engaging with a broad range of health and social care providers. To contribute towards and support the development of quality indicators and metrics to evaluate the impact of the role in supporting service users with complex stroke.
