Maternity Discharge Co-ordinator

Job summary
The post holder is primarily responsible for coordinating the discharge of women and their babies when fit to go home from the postnatal ward, and to help develop a robust process to facilitate and improve discharge.
Main duties of the job
Identify women suitable for discharge and make individual plans for discharge based on the needs of the women, baby & family. Liaise with the midwifery and medical staff to ensure the woman is fit for discharge, and that any further appointments and referrals have been made.
Ensure all necessary screening has been completed prior to discharge and follow up appointments arranged and effectively communicated to the mother.
Ensure all documentation is completed by relevant staff and information communicated to relevant community teams to provide seamless transfer of care.
To provide women and families with information relevant to their discharge from hospital and transfer to community midwifery care.
To act as a source of information for areas such as contraception choices available and infant feeding, providing information on further support available prior to discharge from hospital.
Ensure all women discharged complete FFT questionnaire prior to discharge, and assist with further development of the discharge process.
Continue to fulfil duties of a clinical maternity support worker when workload allows.
