Job summary
An exciting secondment opportunity has arisen for Band 6 Allied Health Professional to join our newly established Sussex Post Covid Assessment and Support Service.
This is a rewarding and challenging post in a new service which has been commissioned to assess, recognise and support patients with Long COVID/Post Covid Syndrome.
Our Post-Covid Assessment Service is a multi-professional team, working in line with national guidance, service specification and standards to offer assessment and support for people with new or ongoing symptoms following an acute COVID-19 episode.
The service offers a comprehensive assessment within 6 weeks of referral, with referrals then made to our internal rehabilitation pathways or externally as appropriate and reviews at 3 and 6 months. We work to ensure that the service is provided with a Personalised Care approach embedded, using Shared Decision Making and Health Coaching, ensuring flexibility is provided as appropriate and focusing on 'what matters to me'. Approaches for rehabilitation are focused on developing supported-self management skills.
The service is AHP led and multi-Professional, being made up of Occupational Therapy, Physiotherapy, Nursing, Care-Navigator, Rehab Support Workers and Admin staff, along with medical advisor input.
Main duties of the job
As a Post Covid Assessor within the team, you will be completing Initial Assessments and review appointments. The assessments are in-depth holistic conversation, covering all aspects of a person's life that have been impacted by Post Covid Syndrome. Working alongside the patient you will develop an on-going plan, agree rehabilitation goals, support with the development of self-management skills and complete onward referrals as appropriate.
As part of an ever evolving and developing service, you will be involved in the ongoing development of the service and be encouraged to develop your leadership and professional skills.
You will be part of a very welcoming and supportive team. We also offer student placements and there will be an opportunity for you to be involved in this.
There are on-going opportunities for knowledge and skill development within the service and you will be expected to be actively involved in this.
You will be required to work from home, however regular contact is maintained with fellow team members, with regular meetings and use of video calling.
