Community Learning Disability Support Worker

Job summary

An exciting opportunity has arisen to work as a therapy assistant within the Wirral Community Learning Disability Team (CLDT). We are a multi-disciplinary team who serve adults aged 18 and over who have a learning disability. The role is community based and you will be visiting service users in their own homes and other community settings.

You will workunder the guidance of specialist physiotherapists and occupational therapists, through joint visits and/or lone working to provide person-centred assessments and interventions to promote health and wellbeing. Effective communication and a compassionate approach are essential for this role.

The role includes working closely with our multi-disciplinary team to ensure we achieve best care for service users and their families/carers.

Main duties of the job

  • To triage referrals and complete initial therapy assessments to inform prioritisation.
  • Carrying out observations for complex assessments and feeding back to the lead therapist.
  • Independently carry out treatment activities/programmes at the request of a therapist
  • Collecting, delivering and demonstrating use of specialist equipment
  • Delivery of treatment through group work, alongside qualified practitioners.
  • Support with delivery of specialist training within CWP and to external agencies
  • Documentation of clinical contact and contribution to reports and care plans.

  • Appropriate Learning Disability / mental health related NVQ level 2
  • To demonstrate a range of work procedures and practices base level of theoretical knowledge
  • Completion of or Working towards Care Certificate (CWP In-house 2 day training to be completed within 3 months of start date

Desirable

  • NVQ level 3 in LD / Mental health

Knowledge and Expertise

Essential

  • Clear understanding of working within a Person Centred framework
  • Understanding of positive behavioural support and the importance of consistency in the delivery of support and care
