Job summary
To work as a member of the practice nursing team providing clinical care to patients in the primary care setting as delegated by the senior practice nurse.Carry out all relevant forms of care, without direct supervision, and may be required to demonstrate simple procedures to qualified and/or unqualified staff undertaking training where agreed with the nursing team.
The Trust currently supports the RealLivingWage(RLW) hourly rate as set by theLivingWageFederation and will apply a pay supplement if the national A4C hourly rate falls below this
Main duties of the job
KEY RESPONSIBILITIES:
Clinical
1. To deliver health care to patients in primary care, working, often alone, to an agreed care plan and supervised by the team leader.
2. To deliver care to patients, following practice guidelines and protocols.
3. To support the delivery of long term conditions management programmes of care.
4. To provide health education and opportunistic health screening both to individuals and groups.
5. To provide a phlebotomy service within the practice setting.
Admin /Clerical.
1. To maintain accurate patient records and make clear concise verbal reports to team leader.
2. To input to GP systems patient information as required.
3. To set up and monitor immunisation clinics within the practice setting under the guidance of the team leader.
Communication
1. To liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.
2.To establish and maintain effective communication with appropriate working relationships with colleagues, patients, carers and other health care professionals.
3. To demonstrate an awareness of clinical governance and risk management, and apply to work situation.
4. To be aware of limitations of own scope of practice, and when to involve a more senior practitioner, both in daily practice and in emergency situations.
