Job summary
We are looking for an enthusiastic, caring and compassionate person who is passionate about supporting people with a learning disability and other complex needs, especially during times of deterioration in mental health and behaviours of concern.
This role is within the Intensive Support Function (ISF) of the Community Learning Disability Team (CLDT) which also includes a Clinical Nurse Specialist and Clinical Psychologist. The focus is on providing a person centred approach to individuals who are experiencing difficulties in their lives.
East CLDT has two bases, one in Macclesfield on Lea Bank Close and one in Crewe on Eaton Street, the base identified for the successful applicant is negotiable. However, there is an expectation that the role covers both areas and other areas in the Trust when required.
Main duties of the job
The post holder will be involved in supporting the team to carry out day to day observations as part of assessments and also interventions to enable people to receive the appropriate support they require.
Supporting individuals to engage in activities that are meaningful to them and also be able to model these to staff supporting the person to enable them to provide the best support for the individual.
There is an expectation in working flexibly at times, both in terms of working hours in the day and travelling around the Trust footprint and out of area when required.
