Job summary

This is an exciting opportunity to join an innovative service which supports young people and their families while they wait for their treatment. The Care Navigator Team is embedded within the Children & Young People's Mental Health Service (CYPMHS) Wirral 0-18 team who work with young people presenting with moderate to severe mental health needs.

The work is mostly community based so having access to transport is essential

Main duties of the job

  • Meeting with families and young people
  • Attending meetings with other professionals
  • Developing plans to access community support
  • Developing sound knowledge of community services
  • Supporting young people to access community services
  • Raising concerns with team leads if presentation of young people deteriorate

Qualifications

Essential

  • Minimum A level education with English and Maths at C grade or above at GCSE
  • Secretarial and/or I.T. qualification at NVQ Admin level 3, RSA3 or equivalent level of experience
  • Adult and Child Safeguarding Level 3 training.
  • Equivalent, relevant demonstrable experience

Desirable

  • Basic life support training.
  • NVQ level 3 in Health & Social Care or equivalent

Knowledge and Expertise

Essential

  • Able to motivate other team members to support organisational developments and health promotion initiatives
  • Confident in organising and chairing multi-professional meetings and case conferences.
  • Highly organised and reliable and willing to take responsibility for own actions.
  • Confident in communication methods and able to communicative effectively with a range of customer and provider groups, verbally and in writing.
  • Proficient in the use of negotiation and motivational skills to promote health and ensure services are used efficiently and effectively.
