Cheshire and Wirral Partnership Trust have an exciting opportunity along with Neston and Willaston Primary Care Network for an Advanced Practitioner in Mental Health to join our friendly, hardworking and patient focused Primary Care Network.
CWP and Neston and Williston PCN are looking for an Mental Health Advanced Clinical Practitioner. The Neston and Willaston PCN consists of Neston Surgery, Neston Medical Centre and Willaston Surgery. The practices work closely with the community with the aim of creating a healthier future for our patient population. The PCN has established roles including Physiotherapy, a visiting Paramedic Service, Clinical Pharmacy Team, Mental Health Practitioner, Social Prescribing Link Workers, and a Counselling service. Within the PCN we have a dedicated team involved with initial planning, sourcing, funding and the implementation of these much-needed services.
The PCN's vision is to: Support positive outcomes in the community. Tackling and preventing health inequalities. Improve patient access to primary care. Help to create a more sustainable NHS
Main duties of the job
To exercise advanced clinical expertise, levels of judgement, discretion and decision making in the delivery of clinical care. Be responsible for delivering high quality mental health assessment diagnosis and treatment, promoting self-management, recovery, and wellness within Neston and Willaston PCN.
To be the first point of contact for patients presenting with Mental Health problems.
To be able to manage acute/ urgent mental health presentations.
To assess patient needs and to negotiate a treatment plan.
To proactively manage and review more complex mental health patients and organise appropriate routine investigations.
