Job summary

We are recruiting a Senior Clinical Pharmacist to develop medicines optimisation services within Old Hall Surgery, part of One Ellesmere Port PCN, alongside 2 other GP practices managed by CWP.

You will be passionate about patient care. You will perform face-to-face & remote medication reviews of patients with polypharmacy, especially for older people & those with multiple co-morbidities. You will be the practice expert in medicines optimisation & work collaboratively with pharmacists within the PCN & across other sectors.

Post is part time 18 hours per week at Old Hall Surgery.

Recruitment & Retention Payment:

**If you are an external applicant & successfully appointed to this role you could receive £4,500 in recruitment & retention payments (T&Cs apply)**

If you are successfully appointed to this role, you will receive an initial recruitment payment of £1,500 on commencement in post, in addition to your salaried pay.

If you stay in this role for 12 months, you will receive another payment of £1,500, & a final payment of £1,500 if you are still with us after 24 months.

Terms & conditions apply. If you leave the Trust within six months of receiving a recruitment or retention payment, you will need to repay the instalment amount in full. If you leave between six and 12 months of receiving the payment, you will need to repay 50% of the instalment.

Main duties of the job

Working as part of a multidisciplinary team, you will develop & run processes for repeat prescription reauthorisation, management of medicines on transfer of care & systems for safer prescribing. You will have excellent written & verbal communication skills, experience of influencing others & conducting clinical medication reviews & be able to work autonomously as well as within a team. You will provide leadership on quality improvement & clinical audit, as well as managing some aspects of the Quality & Outcomes Framework. It is an exciting role; you will shape the development of it & you will be mentored by a GP to support your development. You will also have the opportunity to be supported by the National CPPE training programme to develop your skills.

You will have a role in managing long-term conditions, working directly with patients to assess & treat conditions, & promote self-care. You will improve antimicrobial stewardship & medicines safety for the practice.

**We offer a relocation package of up to £8,000**

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire & Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for & you would need to relocate to within 15 miles of the new base.

  • Mandatory registration with the General Pharmaceutical Council as a UK Practicing Pharmacist
  • Minimum of 5 years post qualification experience
  • Independent prescriber or currently working towards qualification
  • Clinical diploma / postgraduate qualification

Knowledge and Expertise

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • An appreciation of the nature of GPs and general practices
